Services

Information

Every individual on services receives case management to assist with supports, connection to resources, and referrals if needed.  Support Coordinator meets with the individual or family as a team and we determine if additional services are needed.  Once approved they work with a DMH contracted provider to receive those services.  Some services include Day Hab, Support Employment, Personal Assistant and Autism Support.  Other services may be available depending on the situation.

Supported Employment

When an individual needs supports to work in the community a Supported Employment provider can provide a Job Coach to assist with finding and applying for a job.  Then once employment is found the Job Coach assists on the job. This is a great opportunity for those individuals wanting to work in the community but need extra support.

Personal Assistant

When an individual needs support to complete certain activities of daily living, a provider can connect the individual with a Personal Assistant to help complete daily tasks.  Tasks can include self-care, meal preparation, shopping, etc.  This is a great opportunity for those wanting to have more independence in their daily life.

Day Hab

When an individual is wanting more group socialization and community involvement then we can connect the individual to a Day Hab program.  At the Day Hab program individuals will work on independent skills, group activities and community outings.

Autism Support 

We can connect individuals with a diagnosis of Autism to a variety of resources.  This can include grants, parent education and training, Parent Support Group and other resources based on need and location.
