New Horizons provides case management services to individuals with Developmental Disabilities.  Each individual receiving services will have a Support Coordinator working with them to identify, locate, coordinate and monitor services that meet the individuals needs.

To request services and go through the intake progress you will need to contact a regional office in your area.  If you are in Livingston, Grundy or Mercer counties contact the Central MO Regional Office at 573-441-6278 and ask for intake.  If you are in the Caldwell county then contact the Kansas City Regional Office at 816-889-3400 and ask for intake.

To qualify for services you must be determined eligible for supports through the State Division of Developmental Disabilities with a diagnosis of Autism, Down Syndrome, Intellectual Disability or other qualifying diagnosis.
